There are many options currently available for approaching safe weight loss. At our office, you will get the benefit of doctor-led care. While there are some “online” programs, we feel there is no replacement for a face-to-face visit and appropriate lab work and physical exams. Using these medications safely is an incredibly important part of your health journey. Additionally, we usually have the lowest prices in the area because as family doctors utilizing our office, we have the benefit of lower overhead and can pass the savings to our patients.
- Can I use my insurance:
While there are some situations in which we can bill insurance, many insurance plans will not cover weight loss medication and visits. Therefore, we have structured this program with competitive cash pricing regarding medication, labs, and visits. When your insurance does not cover the medication, this is a cost-effective choice. However, whenever possible we will utilize insurance coverage for further savings.
- How does the medication work:
Semaglutide:
This medication belongs to a class known as glucagon-like peptide-1 receptor agonists, or GLP-1s. It mimics this hormone in your body which is released in the gut in response to eating. It acts in the hypothalamus, a part of your brain that controls hunger and fullness. It prolongs your sense of fullness. Also, semaglutide slows down the gastrointestinal tract, keeping the food in your stomach longer, keeping you full longer. Originally, this was a medicine for diabetes, and after showing that it was effective in weight loss, it became FDA approved for this purpose.
Phentermine:
This is a stimulant that is taken orally. We will send this medication to your pharmacy, and if you have insurance, it may be covered. It aids in weight loss by reducing hunger and the release of certain brain chemicals (norepinephrine, dopamine, and serotonin).
- What can I expect:
Two 2 of our most utilized medications are semaglutide and phentermine.
Semaglutide:
Most patients with semaglutide lose weight in the first 4 weeks and in clinical trials lost around 12% of their baseline body weight by week 16, and around 16% of their body weight by the end of their first year. It is likely that long-term continuation of this medication is needed to maintain weight loss. Lifestyle with diet and exercise is also imperative for weight loss maintenance. This medication is a weekly injection that is placed under the skin on your stomach. Usually, the dose in increased every 4 weeks until you reach the max dose of medication or the max dose you tolerate without side effects.
Phentermine:
Studies have shown it can help patients lose about 1 extra pound per week. Weight loss is greatest in the first few weeks of use. You may have extra energy while taking this medication since it is a stimulant. In the past, long-term use was discouraged, but now it is felt that this medication may be used safely for longer periods of time. Typically, the dose stays the same throughout treatment.
- Common side effects:
Semaglutide: Because this medication slows the gastrointestinal tract, nausea, reflux or abdominal pain can be a common side effect. Usually this is mild and limited. There can be some lab changes when you take this medication, and we will follow bloodwork as needed to ensure levels stay stable. Even though this medication was originally for diabetes, it does not commonly lower blood sugar in non-diabetes or non-pre-diabetics. Phentermine:
It is not likely that this medication is habit-forming, but this is a commonly asked question. Since it is a stimulant, it can cause high blood pressure (we do not give this to patients with high blood pressure), rapid heart rate or palpitations, and insomnia. - What if I miss a dose:
Semaglutide:
If you miss a dose, take it as soon as possible. However, if it is almost time for your next dose, skip the missed dose and go back to your regular dosing schedule. Do not double doses.
Phentermine:
If you miss a dose (which is recommended to be taken in the morning to avoid the side effect of insomnia) just pick up the next day with the next dose. Do not double doses.
- Medication interactions and who shouldn’t use:
Semaglutide:
If you are not diabetic, age should be >18 to start this medication. Do not use and tell your provider if you have a history of thyroid C-cell carcinoma, medullary thyroid carcinoma, endocrine neoplasia syndrome type 2, current kidney failure, pregnancy or are breastfeeding.
Phentermine:
Ages >18 and <60. Not for use with patients with high blood pressure, treatment for ADHD or other stimulant use, opiate use, while taking certain medications for anxiety or insomnia, history of cardiac problems, pregnancy, or breastfeeding.
